Hu antibody, also known as anti-neuronal nuclear antibody type I, is an intracellular antibody that recognizes the RNA binding protein family expressed on the surface of neuronal nuclei. Most of the PNS patients with positive Hu antibodies were sensory neuropathy and cerebellar degeneration. Ri antigen is restricted to neurons of the central nervous system, and anti Ri antibodies are mainly found in the brain, brainstem, spinal cord. Patients with positive anti Ri antibodies often have paraneoplastic strabismus myoclonus and ataxia, subacute cerebellar degeneration, 1imbic encephalitis, brainstem encephalitis, ophthalmoplegia and others. The corresponding antigens of anti Yo antibody are mainly cerebellar Purkinje cell antigen-1 of 34 kD and 62 kD. Yo antigen mainly appears in Purkinje cell ribosome and rough endoplasmic reticulum. Therefore, anti Yo antibody can lead to cerebellar Purkinje cell apoptosis. The detection of anti Yo antibody has certain clinical significance for the diagnosis of PNS, especially for Paraneoplastic cerebellar degeneration (PCD) with high specificity. The corresponding antigen of the anti PNMA2(Ma2/Ta) antibody is predominantly a paraneoplastic Ma protein. It has been reported abroad that anti Ma2 paraneoplastic syndrome is mainly characterized by inflammation of the limbic, diencephalon, brainstem and its clinical manifestations including memory decline, abnormal mental behavior, seizures. Anti CV2 antibody was first identified and named in a patient with peripheral neuropathy. Anti CV2 antibody can accurately identify the anti collapse response regulatory protein 5 (CRMP5). CRMP5 is a type of developmental regulatory protein. In peripheral nervous system, CRMP5 is mainly distributed in sensory neurons and its surrounding satellite cells, axons and Schwann cells. Simple sensorimotor abnormalities are not common, so they are known as sensorimotor peripheral neuropathy. The clinical manifestations of PNS associated with intracellular CV2/CRMP5 antibodies are diverse. Amphiphysin is an intracellular synaptic antigen. The binding of anti amphiphysin antibody can destroy endocytosis induced by the neurotransmitter secreted by synaptic vesicles at the nerve terminals, thereby reducing neurotransmitters. Anti amphiphysin antibody is considered to be one of the classic specific autoantibodies with diagnostic significance. It has a direct pathogenic effect on the production of target neurons.
